If you are a UK taxpayer and would like your chosen church to collect an additional 25% of the value of your gift from HMRC then you can complete our online form.

Please ensure you select the appropriate type of declaration you would like to make and confirm you have read and understood your responsibility for tax before clicking ‘Submit’.

Your declaration will be securely forwarded to your chosen church and all details will be removed from our systems to maintain confidentiality.
